I've had a long history (18 years) of cluster headaches which I firmly believe were caused by my Moyamoya.

I haven't had any headaches since my surgeries, but I did have "residual" TIA's for months after both of my surgeries.

I wouldn't be too worried with the "occasional" headache, but I would keep track of them to make sure they don't get worse.

I keep track of my occasional TIA's on the calendar next to my computer so I have a good record of exactly when, where, and exactly what I was doing at the time when they happened. If I notice any "trend" going on, I don't hesitate to email Dr. Steinberg about it.

Bottom line, ONE headache wouldn't worry me, but I'd pay close attention to any trends happening.
