Moyamoya patient information survey

I've recently received a letter from Teresa Bell-Stevens at Stanford University asking for our help regarding a paper she would like to present at the American Association of Neuroscience Nurses conference in April 2007.

The goal of the paper is to enlighten neuroscience nurses about what is perceived to be a lack of information available for moyamoya patients in the general medical/nursing community.

Please take a moment to fill in the survey and share the experience of your (or your loved one's) moyamoya diagnoses.  Information you offer hopes to provide future moyamoya patients with more timely information and treatment options when originally diagnosed.

Data from this survey will only be collected until March 15th, 2007 so your prompt attention is appreciated.

Hi DJ,
I received an email from you today about this survey. I filled it out and clicked the "submit" button. It said it would redirect me to the moyamoya.com site...but I received a error message.  Just want to be sure Teresa gets my survey.  I am so happy to respond to such an important topic that we've experienced first hand!  I thank God for your website everyday!

Hi Renee,

Thank you for trying to fill out the survey.  If you got an error after hitting the "submit" button and you didn't make it back to the message board... chances are... it didn't go through.

Since no personal information is asked for, or submitted with the survey to tell if it actually made it through on this side, the best I can suggest you do is fill in the form again.  If you still have problems submitting the form, please let me know!
