EC-IC bypass and STA-MCA bypass
Sep 3rd, 2004 at 2:00am

I didn't know they were the same thing as DJ mentions in the thread developing the survey.   All the reading I was doing was wondering why...they'd mention EC-IC bypass and not the STA-MCA bypass...then other articles would mention the STA-MCA bypass but not the EC-IC.

Further reading of the Penn State Neurosurgery Website's description of MoyaMoya Disease:  An EC-IC bypass creates a "direct" connection between a scalp artery and a brain (intracranial ) artery.  So if the EC-IC and STA-MCA are the same thing...means that is a direct bypass.

And then the EMS or EDAS are indirect surgeries.

Was reading Wayne's post in the Direct and Indirect Bypass Thread.... I kept seeing the EC-IC wondering why the STA-MCA wasn't mentioned.  Now I know why.  And I know the STA-MCA for Cara was very difficult on both sides...he expressed concerned that it may close as it was VERY small....but when Cara had her second surgery 5 1/2 weeks later...he checked the right side (1st surgery) and it was still open.  It sound like the EMS they did also on both sides was much easier.

Sounds like they do one or the other...Do most of you have just one type of surgery done or did many of you have two types of surgeries done??

Warm hugs,  CarasMOM

Carol,

I read in your other posts about your confusion about the surgeries and I'm glad you finally figured it out.  I sort of laughed though because it brought back memories of when I was first diagnosed and surgery was scheduled just around the corner.  This website didn't exist at that point but I had (luckily for me) already found DJ.  Anyway, I believe my surgery was about 2 weeks away and DJ asked me which surgery I was having.  I didn't have a clue!  Never even heard of, nor understood, about direct or indirect surgeries until he brought it up.  I think back now on how stupid and naive I was just putting my brain into some strangers hands without asking such an important question!  But, again, this board wasn't around and the information I did find on the Internet was out of date and confusing.  Plus, when you or a loved one is diagnosed and all kinds of things are happening (tests, scheduling surgery etc.), you tend to just get shell-shocked ya know?  You walk around in a kind of a daze and just don't think of all the things you should be asking or researching.   Afterwards of course, you think of a million things to ask.  You are not alone in your confusion and you'll find that you learn even more every day!

All I can say is thank goodness DJ put this website together for all of us- especially those newly diagnosed, scared and confused!  Thank you Deej!  Kiss

Lisa

Whoops- forgot to add that I had the STA-MCA only.

I had ECIC bypass surgeries (that is what is stated on my medical letters) in 1996 and until today, I always thought it stood for extra-intra cerebral bypass. I also thought that my surgery was indirect. I didn't think that my temporal artery went direct to my mid cerebral one - I thought that it was plugged into another artery on or near the surface of my brain that encouraged further vessels to grow towards the centre. I think that ECIC bypass sounds like a more broad term used whereas STA MCA actually states a specific artery being used ie Superficial Temporal Artery to Mid Cerebral Artery. Could that mean that ECIC bypass doesn't neccesarily mean that it's direct? ie not always the case that it's connected up to the Mid Cerebral Artery and so that's why my surgeries are still called ECIC? Was just wondering. I'm still learning!

Hi, everyone

I was just reading and thought I'd share.  I am going in for EC-IC surgery.  I was explained that it is a direct bypass that will be completed to correct the blood flow on both sides.  I printed the entire set of questions from DJ's list and added quite a few of my own.  I'm a very detail-oriented person.  I wanted to know it all!!!  I will go in for surgery @ 8 am and Dr. Cassell said to expect it to last at least 15 to 20 hours.  Ugghh.  did this freak anyone out that it takes this long?  I mean, certainly I don't want some exhausted surgeon still working on my brain @ 2 in the morning!!!  Can anyone share with me about how long their surgery took?  Thanks.

Hi Lee!

My daughter had EDAs on both sides under the same anesthesia (sp) almost 2 years ago and it took  15 hours! She of course didn´t suffer from the amount of hours but her parents and the rest of the family were nervous wrecks! Shocked Smiley The doctor explained to me in advance that the critical parts of the anesthesia are "going down" and going up" so the long time is not really an extra risk. It´s actually more risky to have two surgeries if you´re stable medically. All went fine and I´m sure you will be too. At least you won´t notice the long time while it´s going on..
Lots of good luck wishes are coming your way from Sweden!! Smiley

Hi again!
Sorry, I forgot to mention that several surgeons work together so that they can get some rest and food during the surgery, the most of it is routine work once they decided how to perform the surgery and chose methods. Again, good luck and take care

Hey Annica,

Thank you for the 'comfort'.  My neuro-specialist mentioned that there would be three different teams during my surgery, but I didn't realize that might mean surgical teams.  I hope Louise is doing well post-surgery.  I found out my surgery date will be Oct. 14, 2004 today.  Now that I have a date, it seems more real.  Before this it was just 'something' I had to go thru.  Now, it's the real thing.  I'm actually starting to get nervous already.   Embarrassed  Anyway, thank you for your reply and 'happy days' to you and Louise.

Daisy,

Where will you be having your surgery?  (and don't you dare say "on my head!").   Smiley  We don't have a confirmed date yet, but most likely will be in Boston for angio on 10/7 with surgery on 10/11.  If, by any chance, we are in the same place around the same time, we should at least say hello!

The doctor's office is supposed to call us Tues or Wed with a definite schedule.
