Moya Moya symptoms after heart bypass syrgerie

Hello,

I'm brand new to this post and I've been trying to get information on something that has plagued me since 1992.

I had open heart surgery to correct an aneuryizing aora (almost to the arch), and also to replace my aortic valve.  There were complications, and I required a pacemaker.

About a week after the surgery, when I went home, I lost sensation in the right side of my body for about 15-20 minutes.  The doctors told me they thought my INR might not be low enough, so they increased my warfarin to 7.5. 

Still, I got the spells.  Same side of my body, lasting 5-15 minutes.  Numbness, I could still move my right arm and leg, but I could not feel them.  This went on for years.  All the while I also sufferred short term memory loss that never returned from the surgery.

About 2 years ago, I took time off of work, to finally get things addressed.  My neurolgist said he found slowness in my left temporal lobe (please forgive the myriad of typos) and that was probably the cause of the bulk of my TIAs.  So, he put me on an anti-ceasure medicine, I'm sorry the name elludes me at the time. 

A year ago I suffered a stroke, loss the ability to move my right eye past a certain point, and had virtually no control over my right leg and arm.

I am wondering if Moya Moya is something that can be caused or if it is more of a disease.

Any information would be helpfull.  Thank you.

I am not sure what an aneurizing aorta is, but my daughter's moyamoya symptoms worsened after she had surgery to correct a congenital heart defect (coarctation of the aorta).   The moyamoya had been present, and causing problems, before, but had never been detected.   Two weeks after Daphne's heart surgery, she suffered two strokes, and the moyamoya was discovered on an MRA (MR angiogram).   Her doctors have theorized that the heart defect had been causing her to have an elevated blood pressure, which kept bloodflow perfused to her head somewhat.  After the defect was corrected, and she was placed on blood pressure medicine, her blood pressure dropped, causing less blood to get to her head, and the subsequent strokes.

Have you had any imaging studies (like an MRI or CT scan of your head)?   The best test I can recommend for initially looking for moyamoya is an MR angiogram of your head... it is similar to an MRI, but a contrast dye is also used so that the vascularization of the brain can be studied.

The symptoms you are describing do sound similar to moyamoya.  I'm sure there could be other causes, but I definitely think this is something to take seriously and to look for a cause.  Anti seizure medicines only mask the symptoms - they don't address whatever underlying cause there might be.

Thank you Jenny, I do wish the best of luck with your daughter, I can't imagine going through all that with a child, my thoughts and prayers to you and your family.

Unfortunately, as a result of my pacemaker and my artificial heart vavle, I cannot have an MRI, I do not know of the other test you mention.  Can you direct me to more information about it?

Scott,

If you cannot have an MRI, you probably can't have an MRA, either - they are very similar tests - but there may be other tests, such as an angiogram of the head, or specialized CT scan. 

Since you have already suffered TIAs and a stroke, I think you should try getting in touch with Dr. Steinberg (the moyamoya specialist at Stanford), and see if there are tests that he would recommend, given your circumstances.  You can email him at gsteinberg@stanford.edu.

Even if you could not go to see him, getting his opinion is free, and he may have some ideas for you.

I would definitely try to contact him as soon as possible.  If it does turn out to be moyamoya, it is unfortunately progressive -- but it is also very treatable, and symptoms often improve after treatment.
